In Preparation for Your Appointment

At the bottom of this page you will see links to the "New Patient Intake Form". Feel free to print it off, fill it out, and bring it to your appointment. If you are unsure about any questions on the page, we will discuss them when you arrive.

If you are being seen for INFERTILITY, please also fill out the "Infertility Supplement Form" with the help of your partner.

Filling out the form in advance will give you time to think about the things which are important to YOU, but also important for us to know.
​
If you do not have access to a printer or cannot do the form in advance, please do not worry. We will give you a copy on your arrival to fill out. Please expect the form to take 10 minutes or so to complete.

On the Day of Your Appointment

Please plan to arrive at your appointment 10-15 minutes prior to your scheduled time. Parking is $1.75 per hour and can sometimes be difficult so please allow a few extra minutes in case the building is busy on the day of your appointment. We strive to be punctual and have you on your way in a timely fashion. However, there are occasional instances where a delivery (or other emergency) may delay your appointment.

We start with a conversation about what it is that brings you to the office. Following our conversation, you can expect a focused physical examination. For women with gynecologic concerns, this often requires a pelvic examination. This is similar to the "Pap test" you probably have already had at some point with your family physician. You will always be given the option of which parts of the exam you wish to have completed. However, please keep in mind that it can be difficult to come to a complete medical answer for gynecologic concerns without having completed a pelvic exam.

Should I Still Come to my Appointment if I am on my Period?
In general, the answer is YES. We can still move forward in coming up with a solution to the issues at hand. There may be specific situations in which we suggest re-scheduling. If you are uncomfortable with coming because of your menstrual cycle, please re-schedule with as much advance notice as possible. And as always, please call the office directly if you have any questions.
